Patients should self certify for sick leave under 7 days and a form can be downloaded below. For sick leave 7 or more days patients should book an appointment.
Less than 7 days Self Certification
If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self-Certification form yourself.
However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £20.
More than 7 days Sick / Fit Notes
If you have been ill for more than 7 days, please book an appointment to speak to a doctor.
Extending a Sick / Fit Notes
If you have already had an appointment and still not fit to return to work, you can request an extension via reception or complete the online form below.
Please note
- The doctor may decline your request and you will be notified to book a GP appointment
- The practice aims to process requests within 5 working days
- Please be reassured that a note can always be back dated if required
